Start Your Visit to Hudson Dermatology in NY Now
You can avoid filling in and scanning a lengthy new patient form by entering your information online through your secure Patient Portal.
If you enter your information in the portal, you’ll be asked only to read and sign a brief set of consent forms when you arrive for your appointment.
Log in to the portal using your username and password, then click on My Health at the upper left of the browser window. Enter as much information as you can, from the Contact Info section through Quality Measures. For a detailed explanation, see our Portal Guide.
